Scholarship Application for Printing

Criteria for Scholarship

The following criteria will apply to this scholarship. The applicant must:

  1. Be accepted into or actively attending a Practial Nursing Program.
  2. Provide evidence for financial need by filling out the Free Application for Federal Student Aid (FAFSA). Attach the award letter to your scholarship application with the Estimated Family Contribution amount (EFC).
  3. Type a three page double spaced essay with title page and references cited on "The Scope of Practice for the LPN." A Selection Committee will read these essays and choose the top two according to spelling, grammar, format, and reference cited following APA format.
  4. Currently maintaining a cumulative grade point average of 2.50 or above based on 4=A; 3=B or percentile (please provide range). Any nursing courses must have at least a "C" or above grade.
  5. A certificate will be presented to the recipient(s). The check will be made out to the recipient's school with the recipients name on the memo line. The check will be mailed or given directly to the Practical Nursing Administrator/Coordinator of the school. The monies will first be applied to the student's outstanding balance with any remaining monies awarded to the recipient. The Scholarship monies should be used for educational expenses. (If the scholarship monies are used for any other reason, the school shall provide the recipient a 1099 for teh amount of the award.)
  6. Two scholarships will be awarded. The recipient(s) will receive a $500.00 scholarship.
  7. Obtain a letter of recommendation from the Practical Nursing Administrator/Coordinator of the Practical Nursing Program you are attending presently. By signing the letter of recommendation, the Practical Nursing Administrator/Coordinator verfies that the applicant meets all requirements of the scholarship. The letter of recommendation must be submitted with the application form and the essay before the deadline date.
  8. All requirements for this scholarship are to be received by the Slection Committee Chair by April 1. The applicant will submit the completed application form, essay, and letter of recommendation. This may be completed electronically or by mail. These scholarships will be awarded to the recipient(s) at the June Missouri State Assocation of License Practical Nursing Conference.

Applications may be picked up from the Practical Nursing Program Offices. Return to: Practical Nursing Program Administrator/Coordinator or designated person for each Practical Nursing Program site.
